Speech Language Pathologist (SLP) - [Creston, IA]
Blue Stone Therapy is seeking a Speech Language Pathologist (SLP) to provide therapy services to adult and geriatric patients at [Accura HealthCare of Creston] in [Creston, IA].
This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Speech Language Pathologist is responsible for assessments, care plan development, and implementation of treatment programs addressing dysphagia, swallowing disorders, cognitive-communication deficits, voice disorders, and speech and language impairments.
This position will serve multiple partner communities, including [Corning Specialty Care] and [Bedford Specialty Care], with travel between locations as needed.
What You’ll Do
- Perform speech therapy evaluations and develop individualized plans of care
- Provide skilled speech language pathology services focused on functional and measurable outcomes
- Treat swallowing disorders, dysphagia, cognitive deficits, communication disorders, and voice impairments
- Participate in discharge planning and development of maintenance programs
- Supervise Speech Language Pathology Assistants where applicable and in accordance with licensure standards
- Maintain appropriate patient schedules per established guidelines
- Complete timely, accurate documentation to support quality care and reimbursement requirements
- Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
Requirements
What You Bring
- Master’s degree in Speech Language Pathology from an accredited program
- Current Speech Language Pathologist license (or eligibility for licensure) in applicable state
- Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred
- Strong clinical reasoning, communication, and organizational skills
- Commitment to patient-centered care and professional standards
