The Role You’ll Play

As a Family Nurse Practitioner (FNP) within our integrated care model, you will provide primarily low to moderate acuity medical care to individuals with behavioral health conditions.

This role focuses on bridging the gap between physical and mental health, ensuring patients receive comprehensive, coordinated care that supports overall wellness.

You will work closely with an interdisciplinary team including therapists, case managers, and psychiatric providers to support improved outcomes.


Integrated Clinical Care

  • Conduct physical health assessments for patients across the lifespan
  • Diagnose and manage low to moderate acuity and chronic conditions (e.g., diabetes, hypertension, asthma)
  • Provide medication management, including prescribing and patient education
  • Order and interpret labs, imaging, and diagnostic tests
  • Deliver preventive care including screenings, vaccines, and health education

Behavioral Health Integration

  • Collaborate with behavioral health clinicians and psychiatrists
  • Conduct screenings for depression, anxiety, and substance use
  • Support care for patients where behavioral health conditions impact physical health outcomes
  • Participate in integrated, patient-centered treatment planning
  • Consult with psychiatric providers as needed for complex cases

Care Coordination

  • Work within an interdisciplinary team to coordinate care
  • Support transitions between medical, behavioral health, and community services
  • Advocate for patient needs and access to care

Documentation & Compliance

  • Maintain accurate and timely documentation in the EHR
  • Meet productivity and documentation expectations consistent with CCBHC standards
  • Participate in quality improvement and compliance activities

Who You Are

You are a collaborative, patient-centered provider who values whole-person care and understands the connection between mental and physical health.

You are comfortable working in an integrated setting and partnering with behavioral health providers to support complex patient needs in a lower-acuity environment.

Requirements

 

Required:

1. All employees will be required to submit to and pass a background check and drug screening.

2. Must be licensed in good standing to practice as an APRN in Kansas. 

3. Graduation from a Psychiatric Mental Health Nurse Practitioner program.

4.  American Nurses Credentialing Center certification as a Psychiatric Mental Health Nurse Practitioner is required.

5. Graduation from an accredited Family Nurse Practitioner program.

6. Must have 3 years of clinical experience as a Family Nurse Practitioner.

7. Strong oral and written communication skills using the English language.

8. Basic personal computer skills required. 

9. Results of criminal and all other applicable background checks must meet the approval of Pawnee Mental Health Services.

10. Must have current DEA.

11. Must have current BLS.


Preferred:

1. Experience in community mental health or integrated care settings

2. Experience working with patients with co-occurring behavioral health conditions

3. Psychiatric Mental Health Nurse Practitioner (PMHNP) certification or experience strongly preferred
