Help Bring Care Back to Healthcare!

Our mission is simple but powerful: Bring Care Back to Healthcare. Through concierge-level health navigation and advocacy, we help some of the most vulnerable members of our community access the care, resources, and support they deserve.

We are looking for a Supervisor of Navigation who is passionate about leadership, patient advocacy, and building high-performing teams that make a real impact every day.

What You'll Do

As the Supervisor of Navigation, you will lead and support a team of Navigators dedicated to improving patient outcomes and removing barriers to care.

Working closely with the Director of Navigation, you will ensure the team delivers exceptional support to patients while maintaining high standards of quality, compliance, and compassion.

Key Areas of Impact

Team Leadership

  • Lead, mentor, and support a team of Navigators
  • Provide weekly supervision and guidance
  • Foster a culture of accountability, collaboration, and compassion
  • Address performance concerns and support staff development

Navigation Oversight

  • Maintain a working knowledge of each Navigator’s caseload
  • Assign and review cases based on complexity and workload
  • Support staff in navigating complex situations
  • Ensure compliance with all Gellert Health policies and regulatory requirements

Patient Advocacy & Engagement

  • Support Navigators in developing effective patient care plans
  • Intervene when escalated patient issues arise
  • Ensure patients receive timely, coordinated services

Quality Assurance

  • Conduct chart audits and case reviews
  • Monitor electronic medical record documentation
  • Maintain high standards for compliance and accuracy

Collaboration

  • Represent Gellert Health with referral partners and community organizations
  • Coordinate care across healthcare and community resources
  • Support communication between providers and stakeholders involved in patient care

Staff Development

  • Identify training needs for Navigation staff
  • Provide orientation and ongoing training when needed
  • Support staff in building leadership and professional skills

Program & Process Improvement

  • Contribute to improving Navigation programs and workflows
  • Use data and analytics to improve service quality and patient outcomes
  • Stay informed on best practices in health navigation and patient advocacy

Qualifications

  • Must be 21 years or older
  • Supervisory experience in case management or a related field
  • Associate’s or Bachelor’s degree, or equivalent work experience
  • Experience working with culturally diverse, chronically homeless, or vulnerable populations preferred
  • Valid Arizona Driver’s License
  • Clean 39-month Motor Vehicle Record
  • Reliable four-door vehicle and auto insurance
  • Ability to pass a background check and drug screening
  • Spanish bilingual is a plus but not required
