About the Role

Description

Longleaf Hospice and Palliative Care is locally-owned with offices located in Atlanta, Covington and Roswell, offering end-of-life care to those with life limiting illnesses in the communities we serve. We believe that this very special care is best provided by professionals who live and work in the community they serve. At Longleaf Hospice and Palliative Care, we integrate and address the diverse, seen and unseen needs of patients and their families.


As a 7 On / 7 Off After Hours Registered Nurse, you will:

  • Complete an initial assessment of patient and family to determine hospice care needs
  • Provide a complete physical assessment and history of current and previous illness
  • Use health assessment data to determine nursing diagnosis
  • Triage calls and assists patient/primary caregiver with problem resolution; counsel the patient and family in meeting nursing and related needs
  • Make visits for symptom management and support as needed
  • Modify patient treatment plans as indicated by patients' responses and conditions
  • Maintain current, accurate, detailed reports in accordance with policy
  • Monitor, record and report symptoms and changes in patients' conditions to the IDG
  • Provide health care instructions to the patient as appropriate per assessment and plan
  • Obtain and receive physician orders as required
  • Pronouncement of patients
  • Other duties as assigned

Requirements

  • Current license to practice as a registered professional nurse in Georgia
  • Current and continued CPR certification
  • At least one or two years of recent home care experience; hospice experience highly preferred
  • Good verbal and written communication and problem-solving skills
  • Responsible valid driver’s license; reliable, insured, automobile for making home visits required
