Program Mission 

To recognize and meet the distinctive needs of youth ages 18-24 who are experiencing homelessness by providing stable housing, mental health care, case management, life skills training, and connection to community services and resources with the ultimate goal of assisting all clients in obtaining and sustaining safe and stable housing.



Position Summary 

This position provides individualized case management services to youth participating in the Tenant-Based Rental Assistance (TBRA) and Rapid Rehousing programs. The primary goal is to support participants in obtaining and maintaining safe and stable housing while connecting them to community resources and services. The position is based at the VOA Youth Resource Center and includes community-based work such as home visits and meetings with clients in the community. The case manager will also work to develop housing resources and maintain partnerships with landlords and service providers to support youth housing stability.



Essential Duties 

  • Provide individualized case management services to youth participating in housing programs, supporting them in obtaining and maintaining safe and stable housing.
  • Assist clients in developing and working toward individualized case plans and goals related to housing stability, education, employment, health care, and other supportive services.
  • Conduct regular meetings and at least one monthly home visit with each client to monitor progress, provide support, and assess ongoing needs.
  • Maintain accurate and timely documentation including case notes, service plans, referrals, and program records in required databases and tracking systems.
  • Collaborate with program leadership and team members to ensure program compliance, documentation accuracy, and coordination of services.
  • Connect clients to community resources including medical care, mental health services, employment support, legal assistance, education programs, and public benefits.
  • Support clients in developing life skills that promote housing stability and independence, including budgeting, tenant responsibilities, and navigating community resources.
  • Participate in team meetings, case staffing, and multidisciplinary collaboration to support client progress and problem-solving.
  • Develop and maintain positive relationships with landlords, community partners, and service providers to support housing opportunities for youth.
  • Facilitate life skills groups, workshops, or support groups for program participants as directed.
  • Provide transportation and accompany clients to appointments or services as needed using agency vehicles.
  • Maintain professional boundaries and interact with clients in a respectful, trauma-informed, and culturally responsive manner.
  • Maintain regular and punctual attendance.
  • Perform other duties as assigned.

  • Must be able to qualify for and obtain Utah Department of Human Services Case Management Certification within 60 days of hire.
  • Fluency in both English and Spanish (spoken and written) required.
  • Demonstrates strong communication skills and the ability to work cooperatively with others.
  • Ability to maintain professional boundaries, uphold confidentiality requirements, follow agency policies and procedures, and interact professionally with a diverse workforce, clients, and members of the public.
  • Ability to apply Trauma-Informed Care practices and exercise sound judgment within the scope of the position while seeking supervision when appropriate.
  • Willingness to accept supervision, feedback, and direction.
  • Ability to move throughout the facility and engage with clients for extended periods of time.
  • Driving is required. Must be at least 21 years of age, possess a valid driver’s license, maintain a good driving record, and be insurable under the agency’s liability policy.
  • Must pass Utah Department of Human Services – Office of Licensing background screening requirements for individuals working with vulnerable populations.
  • Proficiency in Microsoft Office and basic computer systems.


Physical Demands and Work Location

  • Ability to lift and carry up to 50 pounds to assist with client move-ins and other program needs.
  • Ability to move throughout the facility and engage with clients for extended periods.
  • Work may involve prolonged computer use.
  • This position must be performed on-site due to the need to provide direct client services, including meeting with clients in person, conducting home visits, transporting clients to appointments, and supporting access to services.
