POSITION OVERVIEW: 

The Behavioral Health Navigator works as a member of an interdisciplinary team to support individuals seeking and receiving services, build working relationships between outside referral sources including Juvenile Justice, solve problems, track care coordination and implement new necessary workflows to build this position. Responsibilities will primarily consist of guiding, assisting and supporting individuals who have mental health, psychiatric, substance abuse, medical, social and legal issues to access needed behavioral health care to help them improve their daily living skills, and assist them in their recovery process. This position involves participation and implementation of program goals and objectives to promote a strengths-based, wellness-informed client-centered approach to assisting individuals in achieving consumer-driven goals. 


The role of the Behavioral Health Navigator is to provide care coordination and reduce barriers for patients accessing outpatient services. Screening clients and conducting American Society of Addiction Medicine (ASAM) assessments, as appropriate, to ensure they are a fit for Outpatient Services and/or psychiatry, communicating with Rimrock Trails Front Office Coordinators and counselors, closing referral loop, and tracking certain metrics regarding care coordination. 


ESSENTIAL POSITION RESPONSIBILITIES: 

  1. Manages and coordinates referrals to specialty behavioral health services in Deschutes County 
  2. Reduces access barriers for patients referred to specialty behavioral health services  
  3. Responds to emails and telephone messages within 2 business days. 
  4. Maintains waitlist availability for Peer Support, Groups, Youth ASAM, and others as requested and follows up with clients no less than one time per month to ensure continued interest and continuity of care.  
  5. Optimizes individuals served satisfaction, provider time, and treatment room utilization by scheduling appointments in person or by telephone. 
  6. Utilizes the Electronic Medical Records System for scheduling 
  7. Enters referral information from email or e-fax into the EMR within one business day of being received 
  8. Comforts individuals served by anticipating their anxieties, answering questions, and supporting the reception area when needed, including coverage at each location and psychiatric support.  
  9. Ensures availability of treatment information by filing and retrieving individual’s served records if applicable. 
  10. Maintains individual’s served accounts by obtaining, recording, and updating personal, medical, and financial information. 
  11. Foster collaboration between outpatient programs and referral sources to smoothly transition clients through the continuum of care. Including outreach efforts to build and maintain relationships.  
  12. Support case management and transition of care activities and help coordinate patient care needs with outside providers and community resources. 
  13. Accumulates all new client information, enters the data including health insurance benefits, describes the intake process in depth to clients, explains insurance benefits, and assists with intake paperwork as needed. 
  14. Is accessible and visibly present to members of the Specialty Behavioral Heath Outpatient Provider teams during clinic hours. 
  15. Is available to primary care patients and health professionals on a same-day basis when possible. 
  16. Manages and coordinates referrals to community resources. 
  17. Maintains up-to-date contact information about commonly used community resources. 
  18. Execute duties of the position, the provision of patient services and collaboration with referral sources in such a way that supports and facilitates quality care and a positive work environment for all. 
  19. Assists in providing interpretation services and supports the clinic in providing culturally sensitive services. 
  20. Assures availability of patient education and other materials related to Outpatient Primary Care services. 
  21. Understand applicable Oregon Administrative Rules (OARs). 
  22. Maintain timely and accurate documentation required by the position in accordance with OARs. 
  23. Adhere to HIPAA laws and regulations concerning confidentiality and security of protected health information. 
  24. Conduct American Society of Addiction Medicine (ASAM) assessments for Juvenile Justice Departments in Deschutes County, as qualified. 
  25. Facilitate community-based skills groups

Pay: $26.44/hour

Requirements

QUALIFICATIONS, SKILLS, and EXPERIENCE: 


Minimum/Required:    

High school degree plus three years’ experience as a billing specialist, case manager, behavioral health coordinator, QMHA/QMHP, or similar.  

 

PHYSICAL JOB DESCRIPTION: 

Equipment Used: 

Telephone system, computer, copier/printer/binding equipment, agency vehicle.  


Essential Physical Tasks: 

The position requires the ability to lift 25lbs on an occasional basis and utilize typical office equipment, including but not limited to a computer, printer/copier, binding equipment.
