Position Summary
The Community Health Worker (CHW) – Health & Benefits Access provides direct client navigation services to individuals and families seeking access to health coverage, primary care, public benefits, and community-based resources.
This role works directly with clients to assess needs, support enrollment in health coverage and public benefit programs, coordinate referrals to health and social services, and promote continuity of care through follow-up and care navigation. The CHW manages an active client caseload and works collaboratively with health plans, providers, government agencies, and community partners to improve access to appropriate services and address social drivers of health.
Services may include support related to Medi-Cal, CalFresh, WIC, Covered California, CalAIM programs, and other community-based resources. This position also participates in public health outreach, enrollment events, and community engagement activities.
This role is non-supervisory.
What You’ll Do
Client Navigation & Care Coordination
- Conduct intake assessments to understand client needs related to health coverage, primary care access, and public benefits.
- Manage an active client caseload and provide ongoing navigation support to help clients access services.
- Assist clients in navigating the health care system, including connecting them to primary care, dental, behavioral health, and specialty services within their health plans.
- Serve as a liaison between clients and providers to support access to appropriate care.
- Conduct follow-up with clients to promote continuity of care and successful service linkage.
Benefits Access & Resource Navigation
- Support enrollment and navigation for public benefit programs such as Medi-Cal, CalFresh, WIC, and Covered California.
- Screen for eligibility and schedule appointments for individuals seeking health coverage or public benefits assistance.
- Identify and address social drivers of health through referrals to community-based services and emergency support resources.
Community Engagement & Outreach
- Participate in community outreach events, benefits enrollment activities, and public health education efforts.
- Build relationships with community partners and service providers to strengthen referral networks.
Documentation & Data Integrity
- Maintain accurate and timely documentation in Salesforce or other approved data systems in accordance with program and Medi-Cal documentation standards.
- Ensure client interactions, referrals, and service outcomes are recorded consistently and in alignment with organizational data policies.
- Support program reporting and data quality efforts.
Requirements
What We’re Looking For
Education & Experience
- High school diploma or GED required.
- Associate degree in social services, psychology, public health, or related field preferred.
- Two (2) years of experience in health care, social services, community outreach, case management, interpreting, or client-focused roles preferred.
- Familiarity with Medi-Cal, Covered California, public benefits programs, or communicated based services preferred.
- Knowledge of Medi-Cal managed care plans, CalAIM programs, and Community Health Worker (CHW) services is a plus.

- Ability to travel throughout Sacramento County and surrounding areas

Must comply with any immunization, health screening, or safety requirements required by partner sites or public health regulations.
Work Environment & Schedule
This is a full-time, non-exempt position, typically Monday through Friday between 8:00 a.m. and 5:00 p.m.
This role includes:
- Office-based work
- Regular travel to community sites and partner locations
- Occasional evening and weekend outreach events (scheduled in advance when possible)
Work may take place in office, clinic, community-based, and outdoor settings.
Physical & Emotional Demands
This position requires sitting, standing, walking, and regular computer use. Occasional bending, reaching, and lifting up to 15 pounds may be required.
The role involves working with individuals experiencing complex health and social needs. The ability to manage emotionally sensitive situations with professionalism and sound judgment is essential.
