About the Role

Description

The Social Worker acts as a resident advocate and plans, organizes, and directs the overall operation of the Social Service Department in coordination with the Administrator and in accordance with current federal, state, and local standards.


Essential Duties and Responsibilities:

  • Develop, implement, and direct all social services functions.
  • Ensure an on-going Quality Assurance program and develop and implement appropriate plans of action to correct deficiencies.
  • Consult with and coordinate social services activities with other departments and implement changes as required.
  • Ensure that necessary forms, reports, and progress notes are properly completed and contain informative descriptions of the services provided and the residents’ responses to the services.
  • Participate in community planning, discharge assessments and discharge plans that meet the needs of the facility, the residents and their families.
  • Coordinates social services programs with the residents and their families, staff members, and community agencies through interviews, consultations and direct involvement in the programs.
  • Provide information to family of the resident’s concerning available support agencies and financial assistance programs to meet the needs of the residents.
  • Maintain resident confidentiality; treat residents with kindness, dignity, and respect, know and comply with Resident’s Rights rules.
  • Monitor social services to ensure that the residents’ emotional and social needs are met; ensure that all personnel are knowledgeable about residents’ emotional and social needs.
  • Performs other duties as assigned.


Requirements

  • Must be a licensed Social Worker in good standing with the state.
  • Must be able to interact positively with residents, families, and regulatory agencies, and work collaboratively with employees at all levels.
  • Must be able to maintain consistent attendance and be willing to work long or irregular hours, often under pressure.

Physical Requirements:

  • Must be able to lift at least 50 pounds and remove assistive devices as needed to clean equipment properly.
  • Must comply with all local health regulations and successfully complete a post-offer health assessment.
  • Must be able to perform the essential functions of the position with or without reasonable accommodation.
  • Physical and Sensory Requirements (with or without the use of mechanical devices):
  • Ability to read, write, and speak fluent English to communicate effectively with residents, families, staff, and community agencies.
  • Ability to perform physical activities such as walking, reaching, bending, stretching, pulling, and lifting.
  • Adequate fine and gross motor coordination to carry out required tasks.

Work Environment:

  • Long-term care facility setting.
  • Exposure to residents with varying levels of care needs.
  • May include evenings, weekends, holidays, and on-call shifts.
