Chronic Care Registered Nurse (RN)


Join our dedicated healthcare team as a Chronic Care Registered Nurse (RN), where you will play a vital role in managing patients with long-term conditions such as diabetes, heart disease, and COPD. This position offers an opportunity to make a meaningful impact on patients' lives through personalized care, education, and coordinated support, ultimately enhancing their quality of life and reducing hospital readmissions.


Key Responsibilities:

- Develop and implement individualized care plans for patients with chronic conditions

- Conduct comprehensive health assessments and monitor patient progress

- Educate patients and their families on disease management, medication adherence, and lifestyle modifications

- Coordinate care with multidisciplinary teams to ensure seamless service delivery

- Monitor and evaluate patient outcomes, adjusting care plans as needed

- Promote adherence to treatment protocols and medication regimens

- Document patient interactions and care activities accurately in electronic health records


Skills and Qualifications:

- Valid Registered Nurse (RN) license in the relevant state or region

- Proven experience in chronic disease management or primary care nursing

- Strong knowledge of long-term conditions such as diabetes, cardiovascular diseases, and COPD

- Excellent communication and patient education skills

- Compassionate, patient-centered approach with strong problem-solving skills

- Ability to work independently and as part of a collaborative healthcare team
