What We’re Searching For
Hometown Home Health is seeking a dynamic individual to join our team in a Home Health Registered Nurse role. This position plays a pivotal role in maintaining top tier patient care.
What You’ll Do
- Coordinate patient care in accordance with the treatment plan created by the physician.
- Perform admission, transfer, re-certification, resumption of care, and discharge OASIS for the home care patient.
- Participate in the patient's discharge planning process.
- Prepare progress notes following visits for patients’ charts.
What You Need
- Must have a nursing degree from an accredited program
- Licensed as a registered nurse within the state of Florida
- Adherence to federal, state, and accreditation requirements, including Medicare and Medicaid regulations.
- CPR certification required (Online certification not accepted).
- At least one year of experience at a home health agency or within a hospital.
