Position: Licensed Vocational Nurse (LVN) - Home Health
Employment Status: Full-Time, Part-Time, or Per Diem
Location: Caseload tailored to your preferred location.

Essential Job Functions and Responsibilities:
Key Responsibilities
Patient Care:
- Provides direct patient care as defined in States LVN Nurse Practice Act.
- Implements a plan of care initiated by the registered nurse.
- Provides accurate and timely documentation consistent with the plan of care.
- Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA,OT, PT, MSW, ST and Dietician) to the registered nurse and/or Clinical Supervisor/ Nursing Supervisor.
- Uses equipment and supplies effectively and efficiently.
- Participate in personal and progressional growth and development.
- Performs other duties as assigned by the registered nurse.

Requirements
Qualifications and Skills:
- Graduate of an accredited vocational nursing program.
- Active licensed vocational nurse in the state of CA.
- Valid California driver’s license and proof of auto insurance.
- At least one (1+) year of relevant nursing experience.
- Current CPR certification.
