Position: Licensed Vocational Nurse (LVN) - Home Health
Employment Status: Full-Time, Part-Time, or Per Diem
Location: Caseload tailored to your preferred location.
(OPO Health is licensed in over 15 counties.)


Essential Job Functions and Responsibilities:


Key Responsibilities

Patient Care:

  • Provides direct patient care as defined in States LVN Nurse Practice Act.
  • Implements a plan of care initiated by the registered nurse.
  • Provides accurate and timely documentation consistent with the plan of care.
  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA,OT, PT, MSW, ST and Dietician) to the registered nurse and/or Clinical Supervisor/ Nursing Supervisor.
  • Uses equipment and supplies effectively and efficiently.
  • Participate in personal and progressional growth and development.
  • Performs other duties as assigned by the registered nurse.

Requirements

Qualifications and Skills:

  • Graduate of an accredited vocational nursing program.
  • Active licensed vocational nurse in the state of CA.
  • Valid California driver’s license and proof of auto insurance.
  • At least one (1+) year of relevant nursing experience.
  • Current CPR certification.
