Join Fairlawn Health and Rehabilitation of Cascadia, a vibrant 82-bed skilled nursing facility with a strong average daily census of 70. We are deeply rooted in our mission of being a FORCE for Good, and we’re looking for a Physical Therapist Assistant (PTA) to join our growing, tight-knit therapy team of five superstar clinicians.


Our facility features a well-equipped therapy gym and private Occupational Therapy Suites, creating an ideal setting to make a meaningful impact on patients' lives.


What You’ll Do:

Under the direction of a licensed Physical Therapist, you’ll support patients recovering from injury or illness by implementing treatment plans that promote mobility, strength, and pain relief.

Your responsibilities will include:

  • Assisting residents with ambulation and physical mobility.
  • Implementing therapy plans involving manual exercises, strength training, and the use of assistive devices.
  • Monitoring patient progress and reporting to the Physical Therapist.
  • Educating and encouraging residents in daily living activities and recovery routines.
  • Documenting care accurately and timely in-patient records.
  • Promoting safety, cleanliness, and adherence to infection control protocols.
  • Supporting continuity of care and the therapy team’s treatment goals.
  • Maintaining legal, regulatory, and accreditation compliance.
  • Staying current with best practices through continued education and training.

What You Bring:

  • Knowledge of anatomy, physiology, and physical therapy techniques.
  • Positive, motivational energy, and a passion for patient care.
  • Strong interpersonal and communication skills.
  • Dedication to patient confidentiality and ethical care delivery.

Education

  • Graduate from an accredited Occupational Therapist Assistant program 

Licenses/Certification

  • Current Oregon PTA license required – We’ll cover the cost if you don’t have one yet!
  • Current CPR certification required

Experience

  • Six-month experience in a long-term care environment preferred
