We are seeking a compassionate and dedicated Family Services Coordinator to support patients and families navigating the challenges of terminal illness. In this role, you will provide counseling, advocacy, and case management as part of an interdisciplinary hospice team, ensuring comfort, dignity, and holistic support throughout the end-of-life journey. What You’ll Do
- Provide emotional support and counseling to patients and families
- Coordinate care and connect families with community resources
- Advocate for patient needs within the care team
- Support care planning aligned with hospice philosophy
- Maintain accurate documentation and communication with the interdisciplinary team

Requirements
- Master’s degree in Social Work (MSW) from an accredited program (LCSW preferred)
- Minimum of 1 year of healthcare experience (hospice experience strongly preferred)
- Strong understanding of hospice philosophy and end-of-life care
- Excellent communication and interpersonal skills
- Current CPR certification
- Ability to meet annual influenza vaccination requirements
- Valid Colorado driver’s license, auto insurance, and reliable transportation
- Must meet company driving standards
