🧠 The Role
As a Pharmacist, you will support home infusion therapies for patients with chronic inflammatory and autoimmune conditions.
You will play a critical role in delivering safe, precise, and compassionate care—combining clinical expertise with operational excellence.
🔑 What You’ll Do
- Compound, verify, and dispense sterile medications and nutritional solutions
- Review prescriptions for accuracy, completeness, and compliance
- Monitor therapy, labs, and recommend adjustments as needed
- Partner with providers, nurses, and patients on infusion care plans
- Educate patients and care teams on medication use and safety
- Manage pharmacy operations (inventory, workflow, documentation)
- Ensure compliance with all regulatory and sterile compounding standards
🎯 What You Bring
- PharmD (required)
- Active Pharmacist license in [State]
- 2–5 years of experience (infusion, sterile compounding, or specialty pharmacy preferred)
- Strong clinical judgment + attention to detail
