SCOPE OF WORK: The Community Health Worker (CHW) will serve as a link between health services and the community to facilitate access to services; develop and provide ongoing case management services with the clients/patients, provide health education services in all eight (8) communities located on the Standing Rock Sioux Reservation.
The ideal candidate has a passion to work with Tribal members, has strong communication skills, and strives to provide hands-on Tribal member support, while providing health education and advocating for their health needs.
KEY DUTIES & RESPONSIBILITIES:
- Coordinate and participate in community health initiatives such as health fairs, assessments, and any health related awareness campaigns.
- Provide direct services and basic (heights, weights, blood pressure, temperature, first aid, blood glucose checks, etc.) screening.
- Conduct home visits to members of the community within the service area in accordance with the policies and procedures of the CHR Program.
- Provides limited Community Health Work/Home Health Aide type services (seniors, at-risk adults, ill and/or handicapped patients/clients) as assigned, designated by care plan and/or stated in CHR Program policy and procedure.
- Assist with delivery of medications and provide transportation to clients to appointments as needed.
- Provide advocacy, education services and facilitate access to health services that may be available within the program.
- Will be required to perform other duties as assigned.
PREFERRED QUALIFICATIONS:
- High School Diploma/GED
- Driver’s License required.
SHIFT: Monday through Friday, 8:00 AM – 4:30 PM
TESTING AND BACKGROUND REQUIREMENTS:
Subject to pre-employment, post-accident, and reasonable suspicion drug and alcohol testing.
Tribal, State, and Federal background investigation and MVR screening required.
HIRING PREFERENCES:
Qualified applicants who are enrolled members of a federally recognized Tribe will receive Native American preference.
Qualified applicants will receive Veteran’s Preference with submission of proof of honorable military service (e.g., DD-214).
