Job Description
Care Coordinator - incorporating Onboarding and Training
Your role as Care Coordinator will be support newly employed carers, carry out interviews, induct and deliver training to new carers who complete their onboard as Care assistants with the Home Instead, You will work closely with our teams based in our offices in Colwyn bay, Buckley and Gresford. Primary base will be in Colwyn bay or Buckley
The role offered as a 4 day - (0.8 Pro Rata) or 5 day (fte) can be discussed.

To ensure that our Care Assistants receive the appropriate and high-quality support.
- Support visits, plan introductions and shadow visits.
- Conduct CAREGiver support visits and client QA’s.
- Update and audit client Personal plans and assist with digital care planning.
- Maintain accurate CAREGiver records.
- Build and maintain positive relationships with potential and existing clients, their families and other professionals involved in their care.
- Support and mentor CAREGivers.
- Maintain regular contact with clients and CAREGivers.
- Ensure planning boards are completed monthly
- Any other duties as directed by your line manager.
- Participate in on-call duties as required. compliance with Home Instead’s Equality, Diversity and Equal Opportunities Policy. approximately 1 week in 7 or 8
- Ensure in respect of employment and service delivery.
- Carry out any other duties deemed necessary for the successful operation of the business.
- Deliver care to our clients where required.
- This role requires travelling in and around North Wales including Wrexham, Flintshire, Denbighshire and Conwy areas.
- Flexibility to support CAREGivers and provide care early mornings, late evenings and weekends where necessary.
Qualifications
- Level 3 Health And Social Care or equivalent qualification or higher
- Drivers Licence and own transport
