Responsibilities:
- Receives referrals from the Access team with identified client needs such as Medicaid enrollment, Veteran Affairs authorizations, and determination of need for interagency services
- Assists clients with Medicaid, Veteran Affairs, or other payer source enrollment
- Completes care coordination assessment understanding the physical, financial and emotional needs of the client, which becomes a strategy for successful transition into community. Serves as the contact point, advocate and informational resource for consumers, care team, law enforcement, family/ caregiver(s), payers and community resources.
- Works with consumers to plan and monitor care: Assess consumer’s unmet health and social needs.
- Contributes to the development of a care plan/treatment plan with the consumer, family/caregiver(s) and providers
- Supports the development of and complete identification of clients’ needs including having a thorough understanding of a client’s health needs, financial structure, housing, and transportation to ensure holistic person-centered care
- Communicates with identified care agencies regarding the client’s needs.
- Ensures the client’s relatives, caregivers and support system are identified and provided necessary information, with appropriate client permissions.
- Educates patient and family/caregiver(s) about relevant community resources
- Facilitates and attend meetings between consumer, family/caregiver(s), care team, payers and community resources as needed
- Connects clients with community resources that will assist clients with completing their transition plans.
- Provides follow-up connection for all clients evaluated to improve use and connectedness to services.
- Coordinates with referral sources and internal resources to ensure timely assessments and to facilitate a timely entry into definitive care
- Assists in the identification of “high risk” consumers (the chronically mentally ill and those with specialty health care needs) with immediate response to those identified needs.
Requirements
Talents, skills, and abilities:
- High School Diploma or Equivalency.
- One (1) year direct work experience.
- Must have, or be willing to obtain, a valid Montana Driver’s License.
- Must have an acceptable driving record and pass a criminal background check.
