Tel Hai has an excellent opportunity for a Social Worker to join our dedicated team in Lakeview Personal Care.  This is a part time position working 20 hours per week. 


The Social Worker will provide professional social work intake/referral, and discharge planning services for Lakeview Personal Care while working as a member of Personal Care's resident caregiving team to provide psychosocial evaluations and facilitate appropriate team responses to residents' needs. This individual will coordinate services to enhance residents' length of stay and placement when residents' needs are no longer appropriate to the Personal Care level of service.  They will also ensure resident advocacy while implementing departmental objectives.

Social Worker responsibilities include but are not limited to:

  • Acts as the family/Responsible Party liaison to team members and service plan implementation.
  • Assists residents with the transition into Personal Care and to different levels of service.
  • Assists residents and families with financial concerns and with applying for benefits and entitlements.
  • Coordinates and maintains an accountable, timely grievance response procedure. 
  • Assists in arranging supplemental services (e.g., private duty, Hospice, therapies) for residents.
  • Completes investigations of allegations of abuse.


Requirements

 Social Worker Qualifications:

  • Degree in Social Work required.    
  • Experience in long-term care or geriatrics is preferred.  
  • Ability to deal professionally and tactfully with personnel, family members, residents, visitors, and the general public. 
  • Must be able to work collaboratively with other staff and independently to formulate case management plans.
