Summary:

The Co-Occurring Addiction Counselor is responsible for providing confidential co-occurring addiction counseling, education, and support to individuals, families, and the community and for promoting healthy lifestyles and healthy choices. Performs psychosocial assessments and provides ongoing direct social work services to patients and families. Identifies unmet psychosocial needs and develops programs to address those needs.

Requirements

Minimum Qualifications

  • Licensure/Education:
  • Must hold one of the following credentials: LPC, LCPC, LMSW, LCSW, or LMFT
  • OR be currently enrolled in a Master’s degree program in a behavioral science field from an accredited university or college, with intent to pursue licensure.
  • Substance Use Disorder Qualification:
  • Must meet Idaho Code requirements for a Qualified Substance Use Disorder Professional.
  • Experience & Skills:
  • A working knowledge of administrative procedures in a Federally Funded Community Health Center (FQHC) is preferred.
  • Proficient computer skills required for maintaining electronic health records and internal communication.
  • Strong written and verbal communication skills.
  • Background Check:
  • Must be able to pass an enhanced background check conducted by the Idaho Department of Health and Welfare Criminal History Unit.

Your Essential Duties:

  • Empower clients through individual and group sessions to build resilience, enhance life skills, and make meaningful changes that support recovery and personal growth.
  • Complete and maintain accurate records or reports regarding the patients' histories and progress, services provided, or other required information.
  • Develop client treatment plans based on research, clinical experience, and client histories.
  • Formulate and document in the medical record an individualized plan of care for the patient in a timely manner, in compliance with departmental guidelines.
  • Compliance with documentation standards set by the state and federal guidelines.
  • Coordinate counseling efforts with mental health professionals or other health professionals, such as doctors, nurses, or social workers.
  • Interview clients, review records, and confer with other professionals to evaluate individuals' mental and physical condition and to determine their suitability for participation in a specific program.
  • Review and evaluate clients' progress in relation to measurable goals described in treatment and care plans.
  • Provide clients or family members with information about addiction issues and about available services or programs, making appropriate referrals when necessary.
  • Coordinate activities with courts, probation officers, community services, or other post-treatment agencies.
  • Willingness to attend and participate in off-site meetings and/or seminars.
  • Complete mental health and substance use assessments, including functional assessments such as CANS and ANSA.
  • Regular and predictable attendance is an essential function of this position.
  • Provide a wide range of social work services, i.e., direct counseling services, resource management, etc. in accordance with departmental guidelines.
  • Knowledge and use of evidence-based best practice therapeutic models.
  • Role modeling, organizational, communication, and guest-relation skills.
  • Other duties as assigned.
