Schedule: Monday–Friday | 8:00 AM – 5:00 PM
Pay: $20–$23/hour (based on experience)
Bonus: Additional $1000.00 sign on bonus offered for candidates with current Complex Rehab Technology (CRT) experience paid out upon completed quarter.

The Patient Care Coordinator serves as the central workflow coordinator supporting documentation, scheduling, referrals, and communication required for patients to receive customized mobility equipment.
This role manages multiple active patient cases simultaneously and ensures required clinical documentation moves efficiently between providers, clinicians, and internal teams.

Requirements
What you'll do:
As a Patient Care Coordinator, you will:
- Collect and verify patient demographics, insurance, physician, and therapist documentation
- Manage patient workflow tracking logs across multiple active cases
- Coordinate documentation between physicians, therapists, ATPs, and billing teams
- Follow up on missing paperwork to prevent delays in patient care
- Schedule appointments for clinicians and Assistive Technology Professionals
- Create and maintain patient encounters in internal systems
- Support chart audits and compliance workflows
- Route incoming documentation appropriately and securely
- Contact patients regarding equipment delivery and satisfaction
- Provide coordination support to billing, customer service, and clinical staff
- Assist with phones and front-office workflow as needed
This role supports the full documentation lifecycle required to deliver medically necessary mobility equipment.

Qualifications
Required:
- High school diploma or GED
- At least 1 year of administrative, scheduling, coordination, or healthcare support experience
- Strong organizational and multitasking ability
- Professional written and verbal communication skills
- Comfortable managing multiple priorities simultaneously
- Proficiency with Microsoft Office (Outlook, Excel, Word)
- Ability to maintain HIPAA-compliant documentation standards
Preferred:
- Associate degree or healthcare coordination experience
- Medical office, referral coordination, therapy clinic, waiver services, or DME experience
- Electronic Medical Record (EMR) familiarity
- Insurance verification or intake experience
- Complex Rehab Technology experience (eligible for enhanced sign-on bonus)
