Position Summary:

The Patient Care Assistant is that member of the hospice interdisciplinary team that assists all members of the team in all aspects of team functions to ensure timeliness and cohesiveness of services provided.




Responsibilities:

The Patient Care Assistant maintains the clinical records of the hospice patients accurately and in accordance with the approved format per policy. It is also the responsibility of the Patient Care Assistant to act as liaison between the other team members and to assist in the day to day management of the patients and families we care for through the hospice program. This position reports directly to the Director of Patient Care Services.


Functions:

  • Opens all clinical records in accordance with approved format, maintains clinical and closes clinical records in proper manner. Completes 7 day audits.
  • Assures both physician signatures obtained on Initial Plan of Care within 5 days of admission. rev. 10/13
  • Enters data into Mumms software as well as visits.
  • Acts as a go-between for staff, volunteers and patients/families.
  • Responds appropriately and professionally to all family/patient, professional and staff phone calls.
  • Sets up transport for LOC changes. Completes and is responsible for faxing paperwork required per facility guidelines.
  • Initiates paperwork for coordinating care plan in LTC facilities. Prepares binders, as needed, for LTC facility charts.
  • Assists with scheduling and staffing as requested. Assures that each patient’s clinical record is complete and that all required documentation is filed in a timely manner.
  • Patient Care Assistant will notify the appropriate team members of patient discharge and close the file per approved format.
  • Orders and monitors supplies and durable medical equipment.
  • Prepares documentation for interdisciplinary team meeting as directed.
  • Accepts field assignments as needed.
  • Accepts on call assignments when requested and scheduled.
  • Other assignments as required.

Requirements

Qualifications:

  • Prefer that the Patient Care Assistant holds an LPN or MA graduation certificate from an accredited school and possess a license in good standing with the state board of nursing in the state where the hospice program is licensed.
  • At least 2 years of recent experience in an acute/sub-acute care setting.
  • Prefer demonstrated ability to perform nursing activities within the L.P.N. or C.N.A. scope of practice and to respond to needs of patients and families who have chosen supportive care.
  • Excellent communication skills.
  • Desire to work as an integral member of the hospice team under the direction of the Director of Patient Care Services or designee.
