JOB SUMMARY: The Patient Navigator participates in the delivery of excellent medical services in a patient centered medical home environment with an emphasis on the prevention of disease by providing outreach and education to Nevada’s communities regarding Nevada Health Link. Patient Navigators are responsible for outreach, education and enrollment for Nevada’s uninsured and underinsured populations. They educate people on a variety of topics, including program eligibility, how and why to purchase, and health insurance terms. They are also able to help people enroll in a health insurance plan through NevadaHealthLink.com. The Patient Navigator may be asked to attend Peer Review Meetings and Quality Assurance/Quality Improvement (QA/QI) Committee Meetings to coordinate improvement in all areas of the clinic with an emphasis on the importance of the individual patient and keeping the patient’s needs at the center of all.
DUTIES AND RESPONSIBILITIES:
SCHEDULE:
- Mon, Tues, Wed, Sat: 7:00 AM – 6:00 PM
- Willingness to work alternating evening clinics (9:00 AM – 8:00 PM)
- Evening clinics occur on Wednesdays and Thursdays
Administrative Duties
- Maintains individual confidentiality on all subjects (name, income, family size, education, race/ethnicity, etc.)
- Verifies individual’s identity and keeps thorough records that are stored under lock and key
- Assists individuals in completing the Nevada Health Link Health Insurance application
- Answers questions that pertain to health insurance (differences between plans, definitions, etc.)
- Explains the difference between Medicare and Medicaid
- Assists individuals in applying for Medicaid if they qualify and educates individuals on the process
- Maintains safe, secure, and healthy work environment by establishing and following standards and procedures; complies with legal regulations
- Updates job knowledge by participating in educational opportunities; reading professional publications
- Attends and actively participates in staff meetings
- Performs all other duties as assigned
Other Duties:

- Uphold Complies with HIPAA and protects patient information.
- The Patient Navigator ensures compliance with all 19 Health Resources and Services Administration (HRSA) Health Center Program Requirements.
- The Patient Navigator is also expected to actively participate in and support quality improvement activities and the clinical practice team efforts emphasize the importance of the individual patient and putting the patient’s needs first.
Requirements
QUALIFICATIONS:
Education Required:
- High School Diploma or equivalent. – required
- Completion of all requirements established by the State of Nevada and Nevada Health Link to become a certified Navigator – preferred
- Maintains all BLS certifications and state licensure – required
- You must possess a valid driver’s license. - required

TYPICAL PHYSICAL DEMANDS:
Occasional standing, walking, lifting, reaching, kneeling, bending, stooping, pushing, pulling and lifting and/or moving up to 25 pounds. Frequent sitting. Manual dexterity. Good reading eyesight and vision for close work. Ability to communicate by speaking and hearing in person or on the phone. Ability to travel to various locations.
