Betsy Ross Nursing & Rehab Center is a 120-bed facility offering excellence in rehabilitation and skilled nursing care in a lovely homelike environment that underwent AN EXCITING LARGE-SCALE RENOVATION & IS UNDER NEW MANAGEMENT!   •  The Director of Social Services serves as the resident advocate and leads the Social Services department in a skilled nursing facility. This role ensures resident rights are protected, psychosocial needs are met, and care plans support dignity, independence, and quality of life. The position also oversees compliance, care coordination, and family engagement while ensuring adherence to Medicare/Medicaid regulations. 
  • Serve as resident advocate, ensuring residents’ rights are protected and understood by staff, residents, and families 
  • Lead and manage the Social Services department and administrative functions 
  • Conduct pre-admission screenings, psychosocial assessments, social histories, and care plans within required timelines (within 14 days of admission) 
  • Maintain and update psychosocial assessments and care plans at least every 90 days 
  • Support residents and families with adjustment to facility life and care limitations while promoting strengths and independence 
  • Facilitate Resident Council meetings, grievance resolution, and family support groups 
  • Act as liaison between residents, families, staff, and interdisciplinary care teams 
  • Monitor discharge planning and ensure smooth resident transitions 
  • Ensure compliance with Medicare, Medicaid, facility policies, and regulatory standards 
  • Oversee documentation and coordination of psychology and behavioral health services 
  • Participate in QAPI, UR/CMI, Safety Committee, and other interdisciplinary meetings 
  • Track advance directives and family involvement in care planning 
  • Stay current on aging services, community resources, and relevant social service regulations 
  • Support staff education on psychosocial needs and resident-centered care

Requirements

  • MSW or BSW from an accredited university or institution of higher learning. A minimum of a Bachelor’s degree in Social Work or a related field, required.
  • LMSW (License Master of Social Work) in NYS in good standing, preferred.
  • Minimum of two years of social work management experience in a healthcare setting, Skilled Nursing Facility experience preferred.
  • Pleasant personality and ability to work with residents, families, and staff from various departments.
  • Proficient in Microsoft Office.
  • Excellent communication and organization skills.
  • Ability to work well under pressure and be flexible.
  • Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, & sometimes challenging residents
  • Work well under pressure & willing to work harmoniously with all staff
  • Covid-19 vaccinations are encouraged but NOT MANDATED; we continue to offer COVID-19 vaccination based on eligibility guidance from CDC
