As a Nurse Practitioner supporting Home Health, Hospice, and Palliative Care, you will:

  • Conduct patient visits across all three service lines
  • Collaborate and participate in weekly interdisciplinary group (IDG) meetings
  • Provide clinical guidance and continuity of care
  • Play a key role in shaping patient-centered outcomes

 Requirements:

  • Graduate of an approved Nurse Practitioner curriculum, MSN required
  • Active APRN or NP license required
  • Minimum of two years clinical nursing experience
  • Valid driver’s license
