Position Summary
The Care Navigator / Non-Medical Case Manager plays a critical role in supporting LGBTQIA+ individuals by connecting them to essential resources and services. This position focuses on holistic, client-centered navigation for individuals seeking assistance with housing, food access, healthcare, mental health services, employment, and public benefits.
This role is also integral to supporting individuals newly diagnosed with HIV and those at risk of falling out of care, ensuring continuity, engagement, and access to supportive services.
With a goal of expanding client service capacity, the Care Navigator will help scale navigation services through direct client support, outreach, and community engagement.
Key Responsibilities
Client Navigation & Case Management
- Provide comprehensive, non-medical case management and care navigation services to LGBTQIA+ clients.
- Conduct needs assessments and develop individualized service plans.
- Connect clients to housing assistance, food resources, healthcare providers, mental health services, employment opportunities, and benefit programs.
- Support clients with healthcare and insurance navigation, including scheduling appointments and understanding coverage.
- Assist clients in obtaining essential resources such as cell phones and transportation support.
HIV Care Support
- Provide targeted navigation for individuals newly diagnosed with HIV.
- Support retention in care for clients at risk of disengagement from HIV services.
- Collaborate with healthcare providers and community partners to ensure continuity of care.
Outreach & Engagement
- Conduct outreach activities and represent the organization at community events.
- Build relationships with community partners and service providers.
- Engage hard-to-reach populations and reduce barriers to accessing services.
Program Growth & Capacity Building
- Manage a growing caseload, contributing to the expansion of clients receiving navigation services.
- Maintain accurate documentation and track client outcomes.
- Assist in training new staff or volunteers as needed.
- Any other duties as assigned by manager
Crisis Navigation & Advocacy
- Support clients in navigating complex and high-stress situations with empathy and professionalism.
- Advocate for clients across systems including housing, healthcare, and public benefits.
