🩺 What You’ll Do

  • Perform comprehensive assessments of patients’ physical, emotional, and psychosocial needs
  • Develop and manage individualized plans of care to ensure comfort and dignity
  • Guide and educate patients and families through the end-of-life journey
  • Collaborate with the interdisciplinary team to deliver holistic care
  • Supervise and support Hospice Aides and LPNs/LVNs
  • Monitor patient progress and communicate changes to the care team
  • Provide compassionate support during critical moments

✅ What You Bring

  • Active RN license
  • CPR certification
  • Valid driver’s license & auto insurance
  • Hospice experience (preferred, but passion for hospice is key!)
  • Experience with HomeCare HomeBase (a plus)
