About the Role

Your Role as a Care Partner 

You’ll deliver this role by:  

  • Developing and reviewing personalised care plans that support customer goals, wellbeing, and independence
  • Coordinating services and managing customer budgets within approved funding parameters
  • Building trusted relationships with customers, carers, families, and health professionals
  • Monitoring customer wellbeing, changing needs, and potential risks to ensure continuity of care
  • Applying wellness and reablement approaches that encourage confidence and independent living
  • Supporting customers to make informed decisions and navigate aged care services and supports
  • Maintaining accurate documentation, care notes, and compliance with aged care standards and legislation
  • Promoting safe, high-quality, and culturally inclusive care that respects each customer’s individual needs and preferences

What We’re Looking For

  • Certificate III/IV in Aged Care or equivalent
  • 12+ months experience in aged care, disability, or community services
  • Strong communication and organisational skills
  • Confidence using digital systems
  • Passion for person-centred care
  • Current Driver’s Licence

Essential Requirements 

  • Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy) 
  • Relevant probity checks required by legislation and WMQ Limited policy. 
  • Current Driver’s Licence and willingness to travel for work
