About the Role

Your Role as a Care Partner

You’ll deliver this role by:

Developing and reviewing personalised care plans that support customer goals, wellbeing, and independence
Coordinating services and managing customer budgets within approved funding parameters
Building trusted relationships with customers, carers, families, and health professionals
Monitoring customer wellbeing, changing needs, and potential risks to ensure continuity of care
Applying wellness and reablement approaches that encourage confidence and independent living
Supporting customers to make informed decisions and navigate aged care services and supports
Maintaining accurate documentation, care notes, and compliance with aged care standards and legislation
Promoting safe, high-quality, and culturally inclusive care that respects each customer’s individual needs and preferences

What We’re Looking For

Certificate III/IV in Aged Care or equivalent
12+ months experience in aged care, disability, or community services
Strong communication and organisational skills
Confidence using digital systems
Passion for person-centred care
Current Driver’s Licence

Essential Requirements

Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)

Relevant probity checks required by legislation and WMQ Limited policy.

Current Driver’s Licence and willingness to travel for work
