We are hiring an experienced Home Health RN to join our evening team serving the Phoenix metro area. This role offers manageable caseloads, strong clinical support, and a predictable schedule — without hospital demands or overnight shifts. 


Pay: $80,000 – $100,000/year + Mileage Reimbursement 

  • Competitive salary based on experience 
  • Paid travel between patient visits 

Schedule 

  • Full-Time, Monday-Friday + rotations every other weekend 
  • Visit window: 3:00 PM – 11:00 PM (Visits scheduled within this timeframe — not continuous bedside shifts) 
  • Paid daytime training before transitioning to evenings

Key Responsibilities 

You'll carry a caseload of home health patients across the valley, delivering skilled nursing visits and managing patient-centered care from start of care through discharge. Day to day, that includes: 

  • Perform skilled nursing visits (wound care, wound vacs, IV therapy, medication administration) 
  • Conducting Start of Care (SOC), Resumption of Care (ROC), recertification, and discharge assessments 
  • Manage central lines (PICC, ports) and perform venipuncture 
  • Develop and update Plans of Care in collaboration with physicians 
  • Educate patients and caregivers on treatments, medications, and safety 
  • Coordinate care with interdisciplinary teams (LPNs, CNAs, therapy, pharmacy) 
  • Complete EMR documentation and support discharge planning 

Qualifications 

Required: 

  • Active Arizona RN license (Compact RN license accepted) 
  • Home health experience required (2+ years of direct patient care strongly preferred) 
  • Graduate of an accredited nursing program (ADN or BSN accepted) 
  • Current CPR/BLS/First Aid certification 
  • Valid Arizona Level One Fingerprint Clearance Card (standard for home health in AZ) 
  • Valid AZ driver's license, reliable transportation, and proof of auto insurance 
  • Current TB test results 

Preferred: 

  • Home infusion therapy 
  • Wound care & wound vacs 
  • OASIS & Medicare documentation 
  • Central line / PICC line management 
  • EMR / point-of-care charting
